Therapy sessions

If you book for therapy I take your name, date of birth, address, contact number and GP information for safeguarding reasons. This can help identify a person if they go missing or if there is a concern for their safety or the safety of someone else. I also take your email address as a secondary method of contact if you choose to use this.

Occasionally during or usually after sessions I will take handwritten notes, which are stored in a lockable box in my office. This is also where the paper records including personal data are stored securely.

Personal data is stored until 7 years after you have completed therapy with me, at which point paper records are securely shredded.

Personal data will never be shared with a third party unless the client is at risk, is suffering harm or is a risk to others.

By booking an appointment with me, you consent to the above. Thanks
